WE
SERVE THOSE WHO
Have
limited life expectancy, regardless of type of illness.
Have
a physician primarily responsible for medical care who will work
with the hospice program.
Have
someone available to assist with care. We will work with your family
to find additional caregiver support if and when it becomes necessary.

within the Priority Hospice service area.
THE
REFERRAL/ADMISSION PROCESS
We
are happy to talk with you about Priority Hospice. Actual referrals,
however, are made by the patient's physician, usually following a
discussion about a prognosis pointing to limited life expectancy. A
Priority Hospice nurse will then visit the patient at home, in the
hospital, or in a nursing home to discuss, with the patient and
family, hospice services. Immediate needs will be assessed and a plan

Services
Priority
Hospice home services are designed to ease pain and alleviate
symptoms experienced with terminal illness and to provide support to
the patient's caregiver. Our staff makes regularly scheduled visits
to the patient at home as well as during hospitalization.
PRIORITY

CHAPLAIN WILL PROVIDE:

support with the patient's religious community

with memorials and funeral arrangements

PROGRAM
Volunteers
are a vital part of Priority Hospice team. Our volunteers may give
the gift of time and talent in a variety of ways by:
Aiding
and comforting terminally ill patients and their families
Offering
support grieving families through telephone calls, notes, or
personal visits
Representing
Priority Hospice in the community as speakers or at special events
Assisting
in our office with special projects, mailings and clerical support
